Description

Fidelity House CRC is a private nonprofit organization that provides community residences and supportive services for over 1200 individuals who face a wide range of intellectual disabilities such as cerebral palsy, Down Syndrome, combined cognitive and physical impairments, and autism spectrum disorders. Staff members provide a wide variety of supportive services including information and referral, case management, behavior management support, parent training and support, workshops, ongoing therapeutic programs and recreational activities. Residential services include 26 community residence programs, adult foster care, shared living and support for individuals who live independently in apartments. Each of our more than 490 employees makes a unique and important contribution to the fulfillment of our mission.

POSITION: Clinician, FLEXIBLE HOURS AVAILABLE! 

REPORTING TO: Director of Clinical Services

PRIMARY PURPOSE: Provide substance abuse, behavioral modification and mental health services for adults with acquired brain injuries living in a residential home.

REQUIREMENTS:

·     Master’s degree in Psychology, Social Work or Substance Abuse

·     2-5 years’ experience working with substance abuse, mental health.

·     Preferred experience with ABI population

·     Experience / knowledge of ABA (Applied Behavior Analysis) and/or PBS (Positive Behavior Supports)

·     Proficient in Microsoft Office (Including Excel, Word, Excel, HCSIS, Database management, and other software applications

·     Excellent organizational and communication skills

·     Availability to work flexible hours to accommodate meeting/training/audit preparation needs

·     Ability to motivate direct line employees and leaders during times of change and growth

·     Driver’s license

KEY ACTIVITIES / Essential Functions (Performance Measurements):

1.    Develop, implement, and coordinate behavior management strategies (Behavior Plans, Guidelines, Restrictive interventions, biopsychosocial, and Positive Support Plans). Ensure data collection is maintained consistently and summarized quarterly.

2.    Coordinate training for staff for specific issues, but not limited to, substance abuse, mental health, trauma, and other co-occurring disorders.

3.    Develop and frequently review relapse prevention / safety plan in collaboration with client and the staff

4.    Oversee and coordinate crisis management supports across ID/DD and ABI programs. Coordinate pertinent communications with other community and facility-based health care providers / networks.

5.    Ensure compliance with DDS, CARF, MH, and MRC regulations and standards. Develop and revise policies to coincide with regulations/standards.

OTHER DUTIES AS ASSIGNED, INCLUDING BUT NOT LIMITED TO….

·     Screen new referrals to identify psychological, behavioral supports or substance abuse services needed           

·     Regular communication with Program Directors and direct care staff in order to monitor mental health status, identify potential problems, and promote preventive strategies                   

·     Plan and assist in behavioral/psychiatric crisis response and follow-up care          

·     Communication with psychiatrists, therapists and other health care providers, as needed, to promote quality behavioral health care for individuals

·     Attend professional meetings and conferences periodically to update and refresh skills and knowledge

·     Provide advice and assistance in developing agency policies which affect mental health and behavioral treatment planning
