We currently have an open position for a Spanish-speaking AFC Program Case Manager in Holyoke, MA.

Responsibilities:

• Build relationships with AFC clients.
• Provide presentations to make sure the community is informed about the Program;
• Become someone consumers get to know and trust; respond to their calls/emails/inquiries;
• Work to ensure an excellent patient experience.
• Support the general overall functioning of the office;
• Effectively communicate with patients/family members;
• Provide ongoing care management and support services to the assigned caseload.
• Conduct initial and ongoing evaluation for the client's appropriateness of AFC services if needed;
• Complete all necessary assessments, data, and forms as required for the AFC program in a timely manner.
• Conduct ongoing qualified settings in compliance with the program's regulations;
• Provide ongoing social and psycho assessment and evaluation if needed;
• Assist with all aspects of social services.
• Provide on-call support for clients.
• Participate in team meetings, supervisory meetings, or agency meetings;
• Other duties as assigned.

Education and Experience Required:

• Bachelor's degree and /or a minimum of two (2) years of experience working with the disabled/elderly ( AFC and other related experience) ;
• Ability to work with clients in diverse populations;
• Flexibility, resilience, and ability to handle stressful environments;
• Excellent time and priority management;
• Cooperative and respectful of other team members; a team player;

Knowledge of the community and resources available in the community

Languages:

Must be fluent in English and Spanish
